Payroll Forms

As part of the onboarding process with the Be In Christ, employers must submit payroll forms. Below are the payroll forms for onboarding, concluding and making changes to employee payroll accounts. Please contact Payroll Services if you have any questions about how to use these forms and they will be happy to help you. These forms are fillable PDF so it is our preference that the forms are completed electronically, rather than printed and mailed or scanned. 

Once completed, send the form to Payroll Services and they will ensure the forms are submitted for ADP payroll submission.


New employees are required to submit each of the two forms listed below and banking deposit information to Payroll Services for processing.

  • Include a void cheque or direct deposit form as issued by your bank.
